To Whom It May Concern:
Pursuant to Chapter 119 of the Florida Statutes, we are writing to request any and all personnel records, employment records, health records, payroll records, workers compensation records, W-2 forms, records of earnings, time lost from work, reason for termination of employment, correspondence, accident reports, applications for company sponsored or offered health insurance plans, application for employment, any and all drug tests, and any and all other records and documents. Also please include any and all internal affairs disciplinary records.
If you refuse to provide this information, Chapter 119 requires that you advise us in writing and indicate the applicable exemption to the Public Records Act. Also, please state with particularity the reasons for your decision, as required by §119.07(1)(e), Florida Statutes. If the exemption you are claiming only applies to a portion of the records, please delete that portion and provide photocopies of the remainder of the records, according to §119.07(1)(d).
If you anticipate that in order to satisfy this request a special service charge as defined in §119.07(4)(d), will be required, please provide a written estimate and justification of same prior to the reproduction of the referenced records. Please e-mail or fax the estimate.
