Request is made for a copy of the following:

  • Any video recordings, surveillance tapes, photographs, witness statements, or other evidence the Employer believes supports the allegations giving rise to Griffin's suspension, or which relate to the discipline in any manner;
  • Any and all documents or other materials, including tapes and recordings no matter how stored  or  recorded,  the  Employer considered,  reviewed  or  relied  upon  in suspending Monique Griffin, including any and all documents referred to in the letter issued to her on August 20, 2019;
  • Any and all rules, regulations, guidelines, employee handbooks, memoranda and/or other documentation that deal, in any way, with the discipline and/or the infractions alleged to have been committed by Griffin;
  • A copy of any and all documents and things that the Department intends to introduce, refer to, or rely on at the Civil Service hearing relating to this discipline;

 If you are not the custodian of records, please forward this request to the custodian of records. If the costs of responding to this request exceed $25.00, please contact me before incurring any costs.

In order to minimize the costs and time required to respond to this request, the documents and/or records may be transmitted in electronic format, or if available on a publicly accessible web-site, a reference to this source would be acceptable.
